Chapter (License) Assessment Application

Dear CTC Friend:

Thank you for your interest in The Children's Theatre Company.
Licensing a Children's Theatre Company production is simple. To obtain a performance license, you must first submit:

1. An Assessment Inquiry form (1)
2. Application for License (2)
3. Signed License Agreement with appropriate fees (3)

All of which will be accompanied by a detailed FAQS (3) as well as a telephone conference date with your sponsoring organization to answer questions and to build unity of vision.

Your information you will then be submitted to the CTC NYC Board of Directors, who will determine the scope of work and implementation time line.

Our Production Kit includes forms and templates designed to correspond to the various phases of pre-production, production and post-production stages. We have also included a CD-R of all these documents so that you may make adapt them to your needs and print copies as needed. In addition to these organizational forms, we will include various supplementary items, such as tutorial CDs, choral and orchestral CDs, sheet music etc.

We are happy to help implement any part of the Production Kit and any phase of CTC Chapter development that you feel you are ready to produce.

The information you submit will be submitted to the CTC NYC flagship Board of Directors, who will be consulted and will continue to guide our collective success.

In the meantime we look forward to providing you with any training, orientations, meetings and review of materials that your community may need.

As you embark on this wonderful journey of service to children through the arts- our goal is to help you develop a realistic plan geared to success within your available human and material resources. The Application includes questions that we will be helping you answer and think through. You may already have concrete answers, or you may wish to engage in more consultation with us or with your community first….but please consider each question carefully, so that we can make the fullest assessment of your needs and goals.

Once your application and fees have been submitted, you will hear back from us within a week with our assessment of your needs and our materials.

As a CTC Licensed Chapter you will:
__ Receive your Production Kit (with Administrative, Production Manuals, Press Reel DVD, CDRom, Choral & Musical Tutorial and instrumental CDs with sheet music, songbooks, scripts, letterheads, business cards, company logo etc.)
__ Your name and e-mail address will be added to the Children's Theatre Company website and a designation of Chapter Host will be added to the Children's Theatre Company Referral page.
__ Be invited to a monthly Chapter Coordinator Conference Call (per your request)
__ Be emailed updated scripts, songs, forms, documents.
__ We will continue to offer your support and resources (including recruitment, training, and consultation)
